Complaint Information Form
The Complaint Information Form (PDF) may be used to notify police administration of alleged wrongdoing by a police officer of the Jackson Police Department. Private Property Crash Form
The Private Property Crash Form (PDF) is intended to provide a self-reporting system for your private property vehicle crash. Please follow the instructions and example when completing the form.
Completed forms should be returned to the Jackson Police Department if the crash occurred within the city limits. If the crash occurred outside the city limits, but within Jackson county, mail completed forms to:

Calling 911
Call 911 and do not use this form if any of the below apply:
- The driver of either vehicle is under the influence of drugs or alcohol
- The collision involves reckless driving
- Personal injuries occurred
- The collision occurred on a public street
- The license plate number of a hit and run vehicle was obtained by a witness
Ride-Along Program
To participate in the Jackson Police Department Ride-Along program, read the Ride-Along Rules and Regulations and complete the Ride-Along Waiver Form and the Ride-Along Application Form. Completed forms may be submitted via email here: Email or dropped off in person to the Service Desk at:
