    The Complaint Information form may be used to notify police administration of alleged wrongdoing by a police officer of the Jackson Police Department. All information on page one should be completed as fully as possible. After completing the form, signing and dating it, mail it to the Jackson Police Department at 216 E. Washington Ave., Jackson, MI 49201. You will receive a letter in the mail acknowledging receipt of your complaint.

    You may use this form to request police reports taken by the Jackson Police Department. An incident number is the easiest way for us to fulfill your request. If you do not have an incident number, please provide as much information as possible so we may present you with the information you are requesting. Please understand some information may not be released, based upon current investigations and protected personal information.

    Once you have completed the form, you may fax it to (517) 788-4126 or mail it to the Jackson Police Department Records Unit, 216 E. Washington Ave., Jackson, MI 49201.

    The Private Property Crash form is intended to provide a self-reporting system for your Private Property vehicle crash. Please follow the instructions and example when completing the form.

    Completed forms should be returned to the Jackson Police Department at 216 E. Washington Ave., Jackson, MI 49201 if the crash occurred within the city limits. If the crash occurred outside the city limits, but within Jackson County, mail completed forms to Jackson County Office of the Sheriff, 212 W. Wesley St., Jackson, MI 49201.

    Call 911 and do not use this form if any of the below apply:

    • The driver of either vehicle is under the influence of drugs or alcohol
    • The collision involves reckless driving
    • Personal injuries occurred
    • The collision occurred on a public street
    • The license plate number of a hit and run vehicle was obtained by a witness
