  Required for quarterly performance reporting on projects using CDBG funds

  This form is typically filed with quarterly reports, but can be submitted at any time. Proper documentation must accompany a reimbursement request, such as: Copies of invoices or receipts, with proof they were paid; Employee’s payroll form, timesheet indicating amount of time spent on CDBG-related work, and copy of employee’s check; Mileage form with employee check; or Any other documentation related to reimbursement for eligible CDBG activity.
